Effective teachers identify [and design] formative assessments that can inform instruction and monitor learners’ progress toward meeting essential learning outcomes.

This Formative Lesson Plan artifact directly pertains to formative assessment targeting "learners' progress toward meeting essential learning outcomes". This artifact completely demonstrates learning target A1 because the lesson identifies 3 specific ways to formative assess (circular check, summarizing, am I a "10"? exit slip) and integrates these into the lesson.

Effective teachers identify [and design] summative assessments that can accurately gauge students’ achievement of essential learning outcomes.

Learning target A2 explains that an effective teacher can create or identify summative assessments that accurately measure students learning outcomes. I believe that this learning target can be greatly identified with the portfolio created in my Greek Unit Plan artifact. The portfolio is created throughout all of the contents of math, history and English. The portfolio in this artifact was specifically designed to identify the students understanding of the material in the entire unit, and also for a summative assessment. The essential learning outcomes addressed by the portfolio can be inferred from the artifact provided below.
